INSURANCE

If you have insurance, we will prepare and submit the insurance claim form to your insurance company.  However, if your insurance company does not pay for any reason, you are responsible for the bill.  You must assure that your insurance company pays in a timely manner.  Currently, this office has providers in almost all insurance panels, including Medicare, Medicaid, Tricare, Medcost, Blue Cross Blue Shield, IPRS (State Funding) and Magellan.  It is your responsibility to verify coverage and obtain prior approval before your first office visit.
